Proximal Femoral Reconstruction for Cemented Bipolar Hemiarthroplasty for Unstable Intertrochanteric Fractures in the Elderly

Abstract
Purpose: This study was performed to evaluate the short term result of the proximal femoral reconstruction for primary cemented bipolar hemiarthroplasty to unstable intertrochanteric fracture in the elderly.

Materials and Methods: We reconstructed proximal femoral bone defect with medial neck fragment fixed with circlage wires and two or three temporary axial K-wires for primary cemented bipolar hemiarthroplasty to selected elderly patients with unstable intertrochanteric fracture. Among twelve cases, ten were available after at least a year of follow up. We performed clinical and radiological analyses.

Results: It took average four Week for full weight bearing for all patients. All but one patient returned to their preinjury level of activity in three months postoperatively. The final VAS for pain is average 4.2. Lesser trochanteric and reconstructed proximal medial femoral fragments were all united. No clinical complication was noted related to the technique except three asymptomatic greater trochanteric non-unions.

Conclusion: The early clinical and radiological results of reconstruction of proximal femoral bone defect with circlage wires and temporary axial K-wires for cemented bipolar hemiarthroplasty to osteoporotic unstable intertrochanteric fracture were satisfactory.
